Withholding Allowance Certificates

The following forms allow employees to setup or adjust their tax withholding status properly:

  • W-4 form   - Ensures the correct amount of federal income tax is withheld from your paycheck.
  • W-5 form   - Used when you would like to be paid your earned income credit (EIC) in advance. Eligibility requirements are included in the instructions.
  • IT-2104 form   - Ensures the right amount of state tax is withheld from your paycheck.
  • IT-2104-E form   - Used to claim exemption from federal withholding taxes (submission of a W-4 form that also reflects an status of exempt is required).

 IMPORTANT: All submitted forms must be filled out completely, signed and dated! Incomplete forms submitted to the Payroll Office will be rejected, requiring submission of a new form.
