Employer-Provided Vehicle Certification Form
Individuals who have been "permanently" assigned a Cornell-owned vehicle for business use must complete the form below to report personal use.
The IRS requires employers to report personal use of such vehicles on employee W-2 forms. According to the IRS, activities classified as "personal use" include commuting to and from a regular place of business.
